Services include case management by certified Community Health Workers, establishment and coordination of care with a primary care home and assistance with other barriers to obtaining optimal health.

The Self Regional Heart and Vascular Center is holding a community-based Automatic External Defibrillator, or AED, grant program to train and equip community members in case of a cardiac emergency. The program’s goal is to increase awareness of what to do in a cardiac emergency, train more people on how to perform hands-only CPR and to equip various facilities in the Lakelands region with AEDs in case of an emergency.
